What are Health Related Social Needs?

Good health starts with having your basic needs met. 

This eviction prevention program helps eligible Oregon Health Plan (OHP) members remain housed by providing temporary rental assistance, utility assistance, and housing stabilization support for up to 6 months. 

 

This program is specifically designed to support tenants at risk of eviction and is not intended for homeowners. All information collected remains strictly confidential and protected under privacy regulations.

 

Health-Related Social Needs (HRSN) are the essential parts of daily life. Like secure housing, reliable utilities, healthy food, safe transportation, and a strong support system. When these basic needs go unmet, your physical and mental well-being suffers.That is why we focus on helping you secure these basics first. Addressing these social determinants is a critical step in preventing poor health outcomes and promoting long-term community wellness.

What does HRSN mean?

HRSN stands for Health Related Social Needs. These describe factors that impact a person’s physical or mental health outcomes, like unstable housing

How do I know if I Qualify?

  1. You must be an OHP recipient, through a CCO (like Columbia Pacific) or Open Card
  2. Some additional criteria include income, age, and renter status.

How is CHS involved?

CHS is contracted to provide some HRSN services to eligible members of OHP in Columbia County. In you think you may be eligible, contact us!

How long does it take?

Once we receive your request, it takes anywhere between 7 to 21 days before a decision is made. You can help us speed up this process by submitting all your documents at the same time.

This program is not an emergency service and cannot provide immediate relief. If you have an upcoming eviction court date, please contact your local Community Action Team (CAT) immediately (503) 397-3511

What if I don't qualify?

What if I don’t qualify for OHP housing benefits, but still need help? If you are not eligible for the OHP program but are struggling with rent or utilities, you still have options:

  • Call 211: They will narrow down alternative utility and rent relief programs based on where you live.
  • Contact Community Action Team (CAT): (503) 397-3511 to learn about our non-Medicaid local emergency funding and housing resources.
